Terms and Conditions: Flat Klondike Family Contest Terms and Conditions 1. Contest Overview Contest Name: Flat Klondike Family Contest Contest Period: Starts on July 1, 2024, and ends on July 31, 2024 Voting Period: Begins on August 1, 2024, and ends on August 14, 2024 Winner Announcement: August 15, 2024 2. Eligibility The contest is open to families with children aged 3 to 14. Entries must be submitted by a parent or legal guardian on behalf of the child. Participants must be able to physically pick up the prize from one of the 16 Healthcare Express Urgent Care locations across four states. Healthcare Express employees and their families are not eligible to enter or win. 3. How to Enter Download: Visit gohce.com/flatklondike to download the printable Flat Klondike mascot. Decorate: Decorate Flat Klondike any way the family chooses. Photograph: Take a picture of the decorated Flat Klondike during your favorite summer activity. Submit: Upload your photo using the submission form on gohce.com/flatklondike. Ensure the photo file size does not exceed 10 GB. 4. Submission Guidelines All entries must be submitted by July 31, 2024. No offensive material will be chosen to win. Participants may submit multiple photos, but only one photo per participant will be chosen for the voting phase. 5. Judging and Voting A panel of three Healthcare Express team members will select the top 25 submissions based on creativity. These will be posted on Healthcare Express Urgent Care’s Facebook page (https://www.facebook.com/HealthcareExpressUrgentCare) on August 1, 2024. Voting will be open from August 1 to August 14, 2024. The photo with the most comments on our Facebook page will win the grand prize. 6. Prizes Grand Prize: The winner will receive a Nintendo Switch. The winner will be notified through the parent or guardian’s contact information provided during submission. If the winner cannot pick up the prize, it will be awarded to the participant with the second highest number of comments. Only one prize will be awarded. 7. Contact and Notification Parents or legal guardians will be contacted via the information provided in the submission form. If a parent or guardian fails to respond within 7 days of notification, another winner may be selected. 8. Privacy and Publicity By entering the contest, participants agree to have their child’s first name and city shared publicly if selected as a finalist. All contact information will be kept confidential and used solely for contest-related purposes. Healthcare Express’s privacy policy can be found here. 9. General Conditions Healthcare Express reserves the right to modify or cancel the contest at any time. Entries that do not comply with the official rules will be disqualified. Participants agree to allow Healthcare Express to use submitted photos for promotional purposes if consent is provided. 10. Limitation of Liability Healthcare Express is not responsible for any technical issues, lost submissions, or other problems that may affect participation in the contest. 11. Agreement to Rules By participating in the contest, participants agree to these official rules and the decisions of the judges, which are final and binding in all respects. 12.
