Official Terms & Conditions

By participating in the Free Healthcare Express Visits for a Year Giveaway (“Promotion”), participants agree to the following Terms & Conditions:

1. ELIGIBILITY

  • Promotion is open to eligible participants as determined by Healthcare Express (“HCE”).

  • Limit one (1) prize per person.

  • The prize is valid only for the selected winner and is not transferable.

  • Employees of Healthcare Express and their immediate family members are not eligible to win.

2. PRIZE DESCRIPTION

  • The prize consists of free Healthcare Express clinic visits for one (1) year from the date of prize activation.

  • Free visits apply only to services provided directly by Healthcare Express at participating locations.

3. INSURANCE & REGULATORY LIMITATIONS

  • Free clinic visits are subject to insurance eligibility and applicable federal and state laws.

  • Patients covered by Medicare, Medicaid, TRICARE, or TriWest are not eligible to receive this prize due to insurance restrictions.

  • If a winner’s insurance status changes during the prize period, eligibility may be affected.

4. SERVICES INCLUDED & EXCLUDED

Included:

  • Provider evaluation and management services rendered directly by Healthcare Express.

Excluded:

  • Services referred to or performed by third-party providers

  • Send-out laboratory testing

  • Imaging or diagnostic services not performed onsite by Healthcare Express

  • Medications, supplies, or treatments not expressly included

  • Any services deemed medically inappropriate by the provider

All care is subject to provider medical judgment and clinic policies.

5. NON-TRANSFERABILITY & NO CASH VALUE

  • The prize:

    • Has no cash value

    • May not be sold, transferred, substituted, or exchanged

    • Applies only to the original winner

  • No substitutions or credits will be provided for unused visits.

6. TAXES & RESPONSIBILITY

  • The winner is responsible for any applicable taxes or reporting requirements associated with the prize.

7. MODIFICATION OR TERMINATION

  • Healthcare Express reserves the right to:

    • Modify, suspend, or terminate the Promotion

    • Adjust terms as necessary due to changes in laws, regulations, or insurance requirements

  • The Promotion is void where prohibited by law.

8. ACCEPTANCE OF TERMS

Participation in this Promotion constitutes full acceptance of these Terms & Conditions.

All services are provided in accordance with standard clinic policies and provider discretion.
